Copy a new
registration form for each training you plan to attend.

Fill in the workshop
name, date, and cost.
v
Registration form
and receipt of payment are required for registration.
v
Training is $3.00 an
hour unless otherwise noted.
v
Since
pre-registration is required and because space is often limited,
participants must pre-register to ensure a space for themselves at
the workshop.
v
Registration fees
are non-refundable unless the workshop is full or cancelled.
Credit will be given if you contact us 2 business days prior
to the session.
v
Child Care Resource
and Referral maintains that children deserve well-supervised active
play opportunities currently unavailable at these adult workshops.
Participants will not be allowed to stay if they bring
children.
v
Call for
confirmation or provide an e-mail address for confirmation that the
training registration has been received.
v
Registration in NOT
complete until payment (purchase orders will be accepted) is
received.
v
Payment and
registration form must be received a week (7 days) before the
training is to occur.
v
Confirmation will be
given when payment is received.
